"Treatment of knee and hip osteoarthritis with acetaminophen (Tylenol) is ineffective for reducing pain and improving function, according to a paper published in Lancet (2016 Mar 17. doi: 10.1016/S0140-6736[16]30002-2).
Diclofenac 150 mg per day is the most effective nonsteroidal anti-inflammatory drug (NSAID), BUT IT'S POTENTIAL ADVERSE SIDE EFFECTS must be weighed against other NSAIDs in prescribing decisions, according to the authors.
The study is the largest meta-analysis to date to examine randomized trials of pain-relieving medicines for osteoarthritis.
Dr. Lattermann noted that “the majority of side effects are gastrointestinal. All NSAIDs can cause ulcers and ulcerative bleeding, and they can also be badly tolerated by some patients because they do attack the inner lining of the stomach.”
Naproxen, he said, carries the highest risk of the NSAIDs examined in the study, followed by diclofenac.
Additionally, patients with previous cardiac incidents or atherosclerosis are at risk for heart attack, particularly if they have atherosclerosis of the pericardiac vessels, said Dr. Lattermann."
